can sister and brother breed

i dont want this to happen but can a brother and sister blue and gold macaws breed and if they do will they have demented babies?

The brother/sister, mother/son, father/daughter bloodline is so close as to produce dementia, physical abnormalities, and these abnormalities can be outside or inside or both. To put it bluntly, you will see babies looking like they've been poisoned with radiation. Parents would probably kill every one of them which is a very bad way to start birds breeding because it could become a habit. If parents did not kill the babies then you'd need to take them to Avian Vet to be euthanized. Believe me, you don't want to be a part of this kind of freak show.

If you have a brother and sister pair, you will need to separate them into two cages. HQ also makes a double Macaw cage which has a divider in the middle enabling people to keep both birds in the same cage and close to each other but not allowing breeding. I have my two Amazons in one, but it is actually kind of small for a bird as large as the Macaw. Another cage would probably work better and they can be kept close together. If they are still babies, this is not an issue yet, but you do need to separate them now so it is not so hard later. Macaws come into maturity around 7 years maybe a bit earlier, so be on guard and have them separated way before then or you'll have problems with behavior you don't want to deal with.

Your question is a very intelligent one, and thanks for being smart enough to ask it. Now, take care that this never happens with your Macaws because all babies will be born with various horrors you don't want to see.
