Deworming and vitamins are not necessary. This bird needs to be put on pellets. Here is a link to an article that will help.

How do you know the bird has worms? Has it been taken to an Avian Vet to be examined? If bird has not been seen by an Avian Vet, then it is being killed by being given over the counter poison that has not even been diagnosed that bird needs.Never give over the counter anything to a bird. Unless you are an Avian Vet,you have no way of knowing what is wrong with bird. Pet store people also do not know what is wrong though they will sometimes suggest this or that. Never give over the counter medicines to a bird, and a good healthy diet of organic pellets will stop the need for vitamins at all. Harrison's is the best. All seed diets keep birds half sick all the time and eventually kill them.

This bird needs to be seen by an Avian Vet in your driving area immediately. This sounds like a very bad infection getting worse because of the worm medicine and vitamins.
