what does it mean when my cockatoo fans out its feathers and moves them back and forth on my hand

I don't know that it necessarily means anything. Depending on age of bird, this could be part of a mating display. Males will spread their tail feathers, hold their bodies rigid and walk back and forth on a perch. If your bird is 6-7 years old or older, this may be some kind of display.

If bird is behaving odd in any other ways, then he could be sick. Signs of illness are listlessness, lack of appetite and not talking or playing as they were doing. If your bird shows any of these signs, he needs to be seen by an Avian Vet.

All birds need to be examined routinely at least once a year to catch infection/disease or other physical problems before they get out of control. Birds cover their illnesses and so won't show a lot of the signs I mentioned until they are ready to fall off the perch. This is a protective mechananism. All parrots are still exotic wild animals, and their behaviors will mirror the behaviors of their counterparts living in the wild.
